I understand that Brazilian Jiu Jitsu, mixed martial arts, and related training activities at Arte Suave Jiu Jitsu Academy involve physical contact, strikes, throws, joint locks, chokes, and other movements that carry an inherent risk of injury, including serious injury or death.
I voluntarily assume all risks of participating and agree to release, waive, and hold harmless Arte Suave Jiu Jitsu Academy, its owners, instructors, staff, and other participants from any claim arising out of my participation, to the fullest extent permitted by law.
I certify that I am physically able to participate, and that any medical conditions have been disclosed above. In case of emergency I authorize staff to seek medical treatment on my behalf.
